HOW TO UPLOAD PHOTOS FROM FLICKR TO WORDPRESS BLOG

W-9 FORM
There are 4 pages in this document. You only need to fill out the first page- the other pages are just detailed instructions. Here’s the information you need to include on:

1. Name: use the first line, not Business name
2. Check the Individual/Sole Proprietor box, and the exempt payee box to the right (all this means is that we will not be withholding taxes from your stipend*)
3. Address: use your permanent, home address NOT school 
4. Social Security Number Signature and Date

*The stipend will be issued in two installments over the course of the year. More detailed information will be given at the orientation.

DIRECTIONS FOR THE APPEARANCE RELEASE FORM
Please note that there are two forms - one for minors and one for adults.

• If you are under the age of 18, download the From the Top Appearance Release Form for Minors. Please read the document along with your parent/guardian and then sign and date. Both your signature AND your parent/guardian’s signature are required.

• If you are over the age of 18, download the From the Top Appearance Release Form for Adults. Please read the document, sign your name and the date.
